Just to make sure, on the document checklist, it says that the sponsor must provide a photocopy of EITHER your PR card, Canadian citizenship card, Canadian birth certificate or Canadian passport. I am assuming that I only need to include ONE of these pieces of identification, correct?

Yes. Only one is necessary.
You may want to include one of the first three, plus the passport if you are using the passport to prove visits. (Since then the passport will then be in a different place in your evidence.)

Well I was born in Canada, so I guess I'll just include my birth certificate.
