Sponsor Questionnaire IMM5540

I just couriered in my application last night via FEDEX and of course we start to second guess ourselves with making sure it's complete etc

As for the Sponsor Questionnaire IMM5540 an original copy was enclosed in my first envelope which included all sponsor forms and supporting documents. My concern is when I looked at the Immigrants checklist (South America - Brazil) it also has the same form listed 'must be filled out by your sponsor' should I have included a second identical IMM5540 in my second envelope for sponsored person - supporting documents?

I followed the checklist and this document is only listed once in the sponsor portion but not in the sponsored persons list.
