Sponsor Questionaire - Middle Names

On the sponsor questionaire, it asks for you and your parents' full names. Does this have to include middle names? On my passport I only have my first and last name.

Also, on the Application to Sponsor, it asks for your name to be printed EXACTLY as on your passport. Does that mean I should write it in capitals?

If it asks for full names of your parents, you should put middle names if you can.

As for your name, there is a section where it asks for family name/surname and then given names. You should put your first AND middle name here. However, any section where it says to write your name EXACTLY as it is on your passport, do not include your middle name (because it's not on your passport). You can put it in capitals if you want, it won't matter, they just mean EXACTLY as in spelling, any hyphens, and, in your case, without middle name.
