Sponsor evaluation

Hi,
I have a doubt here if anyone could help me.
I am working right now but my employer does not provide me any T4.
In sponsor evaluation form should i tick employed or self employed???

But i do have a letter from my employer as a proof of employment stating my hours and salary.
Little confused here. I am gonna provide CIC tha 2015 notice assessment of taxation which does not mention anything about T4 since i have never had one.

what should i do with sponsor evaluation form??

Have you asked your employer for a T4? They are legally required to give you one.

Of course, the salary in the T4 also gets reported to the tax authorities.
