On question 9 Form IMM 5540 asks:
Are you a naturalized Canadian or permanent resident of Canada?
My wife (my sponsor). Was born in Canada but, has lived as a resident Alien in the USA since 1968.

So the answer should be yes. The follow up asks on what date did you become a permanent resident?Which would either be date of birth or when we will be moving to Canada?

On question 11 Form IMM 5540 asks:
Are you currently living in Canada? 'No' being the answer. Followed up with When do you plan to return to Canada to resume residency? Which we will put the date we are moving (this June).

The next line asks for proof of intention to re-establish in Canada. Which is tricky. We are moving into a multi-generational house owned by my Mother-in-law. Which we will be living and helped her out.

As such we do not have any of the other "proofs" and need to fill out something in Other. I am not sure what to fill out.

Or we should we say Yes and provide our Canadian Address?

For the plan to re-establish we set out the steps the sponsor had taken and will be taking - researched the job market, bank account in Canada, transferring money into Canada, living arrangements, why that area, etc.

For common law at least we had 2 statutory declarations and on one it was my mother in law confirming we will live there upon initial landing.

Quite a bit may be speculative but write it out to show you have thought about it. We added in our plans for transporting our animals. You may have shipping quotes already?

The answer should be no. Your wife was born in Canada.

Naturalized Canadian citizens are people who obtained citizenship after their birth through immigration.

for your first question :

if your wife was born in Canada and is Canadian, then the answer to that question is no - she is not NATURALISED canadian or PR of Canada, she is Canadian, full stop. if she is a naturalised canadian, it means that she became Canadian later on, she was not born canadian.

Proofs of intention: get a letter from your mother-in-law that you will be living with her when you arrive. Get quotes for shipping stuff to Canada and include them. You can also write a timeline ( we did that), about what you need to do before you leave the US - sell the house, or give notice to the landlord, etc. if you have children and you have started contacting schools for them for september, include that. You probably have discussed with your wife details about how to handle the move - write them down, and show CIC that you are serious about moving to Canada once you get the PR.
