Some questions regarding Online visa extension approval

My mother (currently in Canada) applied for a visa extension using the online system and after a few weeks it was approved (yesterday). She has two PDF messages in her CIC inbox (named "Request letter"). One is a letter stating that her extension has been approved and she has to undergo a medical exam. The second PDF is the medical examination form (pre-filled with her details).

My question is, should we just print out the attached medical examination form, then go to a panel physician or will CPC Vegreville send out actual physical copies of the approval letter and forms?

For the photos, how many will we need and can we just printout the same photograph used for the online application (to required size and photo quality) and use that?

You can take that printout and got to the doc, call the doc or better yet go personally with that printout and ask how many photos they need

it should be no issue

I asked this question a few weeks back and finally got time to accompany my mother this weekend for the medical exam. However, we just received another visitor record that states states that this new one replaces the previous one. The fourth condition "MUST REPORT FOR MEDICAL OBSERVATION AND/OR TREATMENT WITHIN 90 DAYS" has been removed. Does this mean that the medical exam is no longer required?

My other question is, while the Visitor Record does state "THIS DOES NOT AUTHORIZE RE-ENTRY", is it still valid for visits to the US? (She only has a single entry visa).
