I applied for student permit in June 2018 and got approved by the end of July 2018. After 1 week of sending my passport for stamping my status in cic was changed back to "Reviewing eligibility" and I was requested to take a medical exam. But the problem was I already took upfront medical for my application and I could not find any option to re-upload the certificate again. Doing following things solved my problem.
You need to submit a case specific enquiry in the office where your passport is and to the high commission go you country. In my case its Banglore (Passport stamping) and Delhi(High Comission).
Reponse was faster for web form then general enquiries. Don't waste time by calling help desk, they cannot help you.

Choose your country and attach the medical certificate while submitting the form. My issue was solved in 5 days. Hope this helps somebody facing the similar issue.
