SINP OID applicant waiting for BGIP

Hello,
I am SINP OID applicant from India.
Dec 2016 - nominated
Jan 2017 - CIC received
Sep 2017 - Medical Request
Waiting for BGIP and PPR

Question :
1)My medical expires on sep 2018. If i get PPR by july 2018 And i want to extend or land canada in 2019 because of some of health issue in family
What is process to inform CIC that i will land in 2019 and ready to do remedical examination.

2)What is procedure to add the new born child after getting PPR request but you are still not landed in canada.

Please help.Thank you

You can increase time to submit passport
