Simplified Application Querys

Hi,

Myself and my long term girlfriend are just about to submit our simpilified app.

I have a couple of questions and after searching i could not find the answers,

1) I am a self employeed IT consultant do i list my company as my employeer for the years or should i list each job with contract beside them.

2) If something happened and we split during the app process what happens. Could i continue as the principal applicant without her? Very unlikely i know but just want to clarify.
