Hello I live in Pakistan and would like to come to Canada to do my Masters in Biomedical Engineering.

Here are my questions:

1) Do I apply into the school/program of my choosing first or apply for the study permit first?

2) My university application is due in March, and I have not started any applications yet. Is it too late for this year or do I still have time?

3) My and my fiancee wish to get married. Should I get married and then apply for the study permit or apply for the study permit before marriage? (If it matters)

First you need to secure your admission in some college/university and then you will apply for your study permit along with the Letter of Acceptance of some institute.

Usually institutes have THREE intakes in a year [Winter (JAN.), Summer (MAY) and FALL (SEP.)].
You are not late at all. Apply now. You can avail FALL (SEP. 2014) intake.

Study Permit has no dependency in your marriage unless your fiancee is Canadian National. You are free to get married now or later.

Thank you. That really helped.

My fiancee is a Canadian citizen/national. So I should get the study permit first, go to the country and then get married?

Exactly!
Secure your admission, get your study permit and go there.
