When sending belongings via a courier (UPS / Fedex), do you still declare the items on your B4 and B4A forms? Do you state that the were sent by that service?

Unless you want to pay duty and taxes, declare them as goods to follow on your B4.

No, it is not required. You would have to bring the stamped form to clear customs at the CBSA office when the goods arrive in Canada, though.
