CIC will only mail the card to a Canadian address. So there has to be someone you know (and trust) in Canada who can receive the card by mail for you and then mail it on to an international address.

You can do the following
1) buy a mail box address in Canada. Ensure this address is not having any PO number in it. UPS provides one such option.
2) Once you have the address, get it updated in CIC for PR card delivery.
3) Go in for mail box provider that also provides International courier service.
4) keep in touch with the mail box provider(phone, emails) on whether the card has been delivered.
5) Once the card has been delivered, ask it to be couriered to your home address.

I went through points 1-4 successfully. Point number 5 could not be done as there were issued with my PR card. Also, I had to get my mail box renewed when I was in my home country and I was able to do it on phone.

Having said the above and as mentioned in a comment above, there has to be trust.
