Self-employed open work permit

On the "Application to Change Conditions or Extend Your Stay in Canada as a Worker", if I am self-employed, do I put that under name of employer?

I am applying Inland for PR from Quebec and want to keep working after my permit expires. Am I allowed to keep working? I couldn`t find anything about this on CIC or this forum.
