Second Program Study Permit from Outside Canada

Hi, I am looking for some advice or feedback from people who have applied similarly before.

I just completed my Master's from a Canadian University in May 2022 and returned to my home country, India, immediately thereafter. The study permit (SP) obtained for this program has an expiry date of April 2023, and hence, I believe it is still valid. I am now going for a PhD program starting in Fall 2022 at a Quebec University, and have obtained the CAQ for it.

My question is regarding the SP now - should I apply for a new SP for my second program, as I am outside Canada and cannot apply for extension? If so, do I need to complete the medical exam (since I am visiting a designated country, India, even though I have not been here for > 6mos)?
Alternatively, should I notify the IRCC now, of change in DLI and program, take up my new study program under the old SP, and later in Jan-Feb 2023 apply for SP extension? If so, will receipt of grades/transcripts of first program indicate its completion and hence affect my return to Canada (validity of SP being 90 days from completion of program)?

Grateful for any advice based on a similar experience. Thanks a lot!
