I was asked to fill a Schedule A Form IMM 5669. I have made 1 mistake in that. They have asked for my mothers family name and given name at the time of birth and I have submitted her name after marriage. Will there be any implications for this? How can I send a rectified form to them?

raise a CSE and send in the new form. once done send an email so you have written confirmation that they have received the revised one.
