I received this email yesterday and cic has given a week for response.
THE EXACT VERBATIM IN THE MAIL IS AS FOLLOWS:
Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. A newly completed Schedule A with details for Address History is required for the dates June 2016-July 2017 . Please refer to the website below

I do not understand the reason behind this query.I got married in June 2016 and i moved in with my partner and we are living in the same place till date. I have mentioned my address accurately. The only thing is that on my PCC the same address is mentioned but they have omitted the road name. Could that be why there is a query?
There are no gaps in my address section.
Kindly Help!
Also should i fill out the entire form or I should only give the address for the asked duration?
