i am sure this question has been asked several times, but my internet connection is tooooo slow :-\, please help
i received Schedule A request as below :
" Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature. Please provide answers to Statutory Questions and provide the addresses for period between 2004/04-2005/02. "
my questions are :

1- which ones are Statutory Questions in the form ?
2- do I provide address for only this period that they asked for?
3- do i provide answers for other sections ? like education, personal history, membership or association with organization, etc ?

The statutory questions are the ones with Yes and No boxes underneath. They deal with membership in certain types of organizations and with whether you have ever been denied entry, etc., to other countries.

You need to fill in your name, the questions they asked you to fill in, and sign the document. You don't have to fill in the rest, though most people do just to be safe.

I've got another question on this topic. Like the OP, I was also asked to submit schedule A (specifically address history) since 2008. It says I should upload the completed details into my document checklist, but I do not see this option when I log into MyCIC. Therefore, if I send it via CSE, is that acceptable?

They should have put a place for it into your checklist. They almost always do. If they truly didn't, then send it by CSE.

I have had two Request Letters in MyCIC since filing my application. The first one did not have a place to upload it and I sent it by CSE, getting a confirmation a couple of weeks later that it had been added to my application. The second (Schedule A) had a place to upload it, and I did. I think that's the safer way, but if there is no place there is nothing you can do but call them to ask about that or send it by CSE.

Hi guys! Please I need your help!
I get request about schedule A for my husband. He was denied a work permit as there was no valid approved LMO at that time. Upon receiving the answer from the government we booked the earliest possible flight and left Canada (2,5 weeks after).
Question is : in Schedule A which box in question 6 should I tick? I am thinking about D and E. Is work permit a temporary resident visa (box D)?. What about E: „been refused admission to, or ordered to leave, Canada or any other country?” Is the refusal letter an order to leave Canada?
Please help!

Hi Aika, you should put YES for both questions as he was refused and ordered to leave Canada. You don't have to worry much as VO will decide whether the answer is applicable to him or not. But if you put NO then it may harm your application when the answer is YES. So, better to put YES and be safe. Hope this helps.
