After having submitted my application on May 22nd, I received a new note in my account.
Essentially, me (primary applicant) is required to submit following:
Schedule A: A separate and newly completed Schedule A Background Declaration form (IMM 5669) bearing an original signature Please complete it for the period of JUL2004-JAN2007. This must be received by: 2017/07/14
I had 2 questions, am not sure has anyone done this.
1. Should i fill the complete form again with all history of educational qualifications and addresses or should only do for Jul2004 to Jan2007 as this is what is required? I don't want to submit all info again to prevent any confusion
2. For this period, I was working with 3 companies and do have a letter of relieving from them. Should i attach it?
Does someone have any experience on above?
