Does it say error? Open it from your computer. Use Adobe Reader and open it from there.

Download it (you will find the button on top right of the window) and save it even if it won't load the actual form. And when you open it from your computer using Adobe Reader, it should be there.
It happened to me and this is how I was able to get the form. Hope this works for you.

You probably need to download it to your PC. right-click the link above and save to PC then open. Some browsers PDF previewer can't handle the macros in the form.
