Sample passport request letter

Is there a way to find exactly what's written in the passport request letter?

I've received TRV's twice before, but can't find the letter on my computer. Tried googling it but can't find the text anywhere.

Can someone who received it recently paste the text on this thread (of course, anonymize it first)? Or perhaps give me a link where I can read it?
