Hi,

I used several names over the years, nothing extreme between them.

Birth: Hari Mansur (I would say there's no last name here, that's why for my first passport my dad put his name as my last name, and it just continued like that for consistency).

Passport (and EE profile): Hari Mansur Husni (Husni being from my Dad's name Mubarak Husni).

My highschool, college diplomas (all abroad, needing last names) uses Hari Husni because...parents.

I'm looking around this forum and I have questions, please!

1. Do I need to declare all those names "Hari Mansur" (birth), and the other two for passport and education. Do I declare those other two as "Son of" or "Other"?

2. Is an affidavit enough, do I need to supply my birth certificate as well? Which is tough right now, because of the global shipping problem from my country(corona). But I already had it translated (sworn with afadavit) and scanned and emailed to me.

3. I'm looking at USA sites and Canada sites, I'm confused who is supposed to be taking oaths on my claims, USA sites it's usually spouse or blood relative. But in this forum, it's me, I'm swearing the truth on all of this.

Can someone help me with this.

I just found out that in my other thread of "Sibling affidavit" that an affidavit is useless for that purpose, is this useless too?

I will also submit my birth cert and all the documents that I have my other names on. But I never went through any legal name change process. All I did, and what we usually do back home because most people don't have more than one name...come to think of it was also an affidavit stating I have these names. Thats how I got my passport and police certificate, with a name different (adding dad's name as a last name) than whats on my birth cert.
