RQ after application return and resubmission

I have been going through many posts here and found that if the application is returned because of missing paperwork or signature and its resubmitted, that person always gets a RQ.

Is that the case with everyone?

People who have been through this situation, please share their views.

my application has been returned 3rd times because of missing documents. I did my fourth on june and no RQ yet.
