Restoring visitor status medical

Hi i am restoring my status within 90 days to visitor.. i had done medical from physician for express entry documents before 5 months.. so can i write yes i did medical exam and then in checklist it asks me to submit emedical form.. so can i submit dat form which the doctor gave me?

OR
Should i say no i did not do any medical exam?? Pleasee helppp

You should say that you did it and include the copy of the form.
