Responding to ITA: Unique Family Situation and addresses

I received an ITA in the most recent (89th) draw. My parents had divorced when I was very, very little and my dad re-married. I was brought up by my dad and step-mother. I don't know anything about my birth mother, not even where she lives. For all practical purposes, my step-mother is my mom and my half-brother is my brother. Do I absolutely have to list them as step-mother/ half-brother in my ITA response? Or is it ok to list them as mother/ brother? In the former case, is it ok to also tick 'all family members have been mentioned here' or with CIC also ask for details about my bio mother?
Secondly, I have lived in many different addresses in the last 10 years. How precise do the addresses lived-in records have to be? I don't remember every address exactly.
