Required to file for taxes or not? Where to get Certificate number?

So I'm filling out my citizenship application at long last, and I'm almost done filling it out, but I got two questions to ask.

Firstly, on the top right hand side of the first page, it asks for the UCI and certificate number. Do I need to fill this part out?. I have my UCI number, but I don't know what my Certificate number is, where can I find it?

The second question is about filling out taxes. I am not sure if I was required to file for taxes. I did file for taxes in every year I was in Canada, but in 2014 I did sell some stocks in a company that I owned, so I figure I would have been required to file for taxes in 2014, which I did. I looked through the list online as to whether or not I was required to file. Again, I'm not sure. Other than selling some stocks in 2014 the only income I got was from my job and cash gifts from my family. All dividends from the stocks were used to buy more stock automatically. I never sold any property or owned any houses or cars during this time.

So what do I check off at section 12, b)?

You do not have to fill in the top right hand side of first page since its for official use . However in 4 a you need to provide UCI number which you will find on your PR card. If you have filed taxed for the entire eligibility period then mark both sides YES.
