Requesting CAIPS

Hello all
I e-mailed Buffalo to ask if they receive my medical test results and other requested documents but they answer that they don't confirm receiving documents or info.
So now I will ask for CAIPS notes to know if my medical was received or not (done more than 2 months ago), so my question is:
should I include a written consent from my spouse and should I fill the form for my spouse also ?
Last thing: do I have to pay the 5 $ for my self and my spouse ?

Regarding caips, only a canadian citizen can apply. If someone is applying from Canada, no need to pay any amount. If a canadian citizen is applying from outside canada then he/she has to pay $5 (Canadian). The principal applicant will apply it for all the members and include their details in the form.

and do you mean to include 3 forms for a family of 3 members for example ?
By the way in one living in Canada can apply whether being Canadian or not.

yes there has to be a different consent forms (IMM5475) for all the co-applicant above 18 years of age. Only a canadian citizen or permanent resident can apply for it.

Actually anyone in Canada can apply for it, not just Citizens and PRs.

If it is your wife's CAIPS file, then you need her consent. Other family members, No.

I differ from you here, apart from the principal applicant all the adult members of family and spouse need different IMM5475, in my view for a speedy disposal of the application.

Why? There is no personal information in CAIPS other than the applicants and that is what the sponsor is requesting.

Although I become a little bit confused !
Here is my situation: My application is for PR, I am the principal applicant and the application includes my spouse as a family member so if I request the CAIPS for myself only, wouldnot this include the information for all the family members ?
and is there any other way to know that they received my medical and other documents ?

When we search net for CAIPS we only get Lawyers website etc, but no official info. Where we can get official info about CAIPS file.

I took the details from those websites as what is required to apply for caips notes? and received mine without any problem. I didn't applied through any lawyer, I got one of my friend, a Canadian resident, to do it for me. I sent all the papers to him and he merely forwarded that along with his details. Whatever I have told above and in my replies is based on my personal experiance.
