REQUEST FOR MEDICAL EXAM

Hi everyone,

I am applying for a study visa and my application requires "REQUEST FOR MEDICAL EXAM". I am in doubt whether I should submit the IMM5753 (option -Yes, I need) or if they want me to submit the confirmation from a doctor that I have already done the exam.
