Request for IMM 5669

Today I got a request for IMM 5669 (bellow). I have some questions and would appreciate comments very much and help
- Is this good or bad?
- My understanding is that I should only provide Section 8 and 12. Am I correct?
- I have lived many times in Canada for short periods (around a month over the last 5 years). Should I mention all address? My list of address is bigger than the available rows. Can I attach an explanation letter?
- I've kept my home address (in my home country) for the periods of time I lived abroad. Can I provide the home address as one row only?
