Renewal of LMO and other questions

Hello, I need help with the following questions:

My Wife's TWP expires next month and her employer said there are changes to the LMO process and that she needs to call CIC to let them know she wants to renew her TWP as a physician.

Has there been any change to LMO?
Can an employer extend an LMO?
Can she apply for TWP extension without an LMO?

Note: Spousal permanent resident application is already in place.
