Renew PR and Income tax question

i want to renew my PR card
On cic website, the required documents states that i should include" A copy of one of the following:

Other Income Tax assessments issued by Canada Revenue Agency within the past five (5) years"
I am confused, do i need to provide all the income tax assessments from past 5 years ？ or just one of them ?

i just submitted the application it clearly says under additional document proof sec.
you need to send your tax assessments issued by cra for two years within last 5 years.

Hi I have a question here and hope you guys could answer. I arrived here in Toronto last January. A month after I found a two part time jobs. One is in McDonalds as a crew for the weekdays and the other one is Home Caregiving for the weekends. I am receiving a paycheck in McDonalds together with some sort of deductions from tax. The other one is also paycheck but don't have deduction from tax and my private employer is declaring it that his paying me this some sort of amount.
Here's my question:
1. Do I need to declare my earnings on my weekend caregiving job?
2. Do I have to pay taxes on it?
3. Is my weekend job consider as an under the table job?
