Remove condition from work permit

good day everyone
Could anybody help me plz with the right link to submit my application to CIC to remove ( not authorized to work in healthcare ) from my open work permit.
- I am now inside Canada
-I did my medical test and got the reference no
- I submit one application but unfortunately I got a reply that it was not the right one
