regarding police certificates

Hi, I have resided in different cities of my country i.e Pakistan.
Do I have to make police clearance certificates from all the cities or from the permanent address I have i.e from my native town??

You only need one PCC and any you get in your country ideally should cover the entire nation. The police are to check the national database before issuing you the PCC, not just local records.

thanks...i got 1 more problem..
my child original d.o.b is november 2015 but due to certain circumstances i registered her name in june 2016..so her official d.o.b is june 2016,,,will this create any issue during medical exam??

That's a clear misrepresentation which you should beware of and can have adverse consequences. Since you know it is not her correct date of birth, you should immediately get it corrected and obtain the right birth certificate. Remember to be honest in all aspects of your application and your dealings.
