regarding medical examination

hi,

i went to ludhiana doctor for upfront medical examination? he gave me one page document which has "e medical logo"on left upper corner of it. my doctor told to submit this to CHC.

but he has not given me form IMM1017.

PLEASE HELP ME.

also, when submitting study permit application, do i need to submit "Welcome Letter" or only "investment directions" from scotia bank??

Lol I had the same questions. You just have to submit the "investment directions" and just the acknowledgement form which they give you after the medical exam.
