Regarding GCMS notes.

Hey all, I'm filling the consent form for the GCMS notes, and I just have a question.

SECTION A - Name of my wife (The applicant).

Section B - Do I put my name as the sponsor/spouse or leave blank? I'm assuming I put my name.

Section C - I put own my information so the info comes to me.

Section D - Email

Section E - If I do infact put my name in Section B, then I am the one to sign here?

Section F - My wife (the applicants) signature

Section G - N/A.


About sums it up? Sorry, forms like this just wreck my head x.x Thanks all. And for those wondering, I'm just ordering the notes to make sure Moscow really did receive the application since they refuse to answer emails .

Seronic, I feel the same way. My head hurts a bit too. The consent form is terribly confusing! As I mention in my other post. You pretty much need a PHD in English to decipher the form. I am not sure how any new immigrants who do not have a good gasp at the language are able to help themselves on this form. I mean your intention is quite simple; you just want to get information to your sponsorship files. However, when you are trying to fill in the IMM5774E consent forms, you have no idea who is giving consent to who with all the dame section area you are able to put names in. Filling in the web form was another challenge. There are more questions on the forms than answer I can pick out, nothing but frustration for me.

Here's what I did.

SECTION A - Name of my spouse (The applicant).

Section B - I left this blank. The applicant is not seeking the information. You are. So the applicant is the one giving the consent? Is it not? Correct me if I am wrong. Anyone familar with the form can answer?

Section C - I put my information on this part so the infomation reqested is sent to me.

Section D - Email

Section E – I left this blank. The reason being is that I am requesting the information on OUR / my spouse's file? The spouse is not a Canadian citizen, I recall reading some part that you have to be a Canadian citizen in order to make the access to information request. So this can't applied, or could it?

Section F - My spouse's (the applicants) signature

Section G - N/A.

Why so comlicated! "A simple my name is..., I give my consent to ..., I am over the age of 18 will do.

They took the money out of my account, but my husband, the canadian requesting the notes did not sign the consent form .... My me the American applicant did son the form. We both looked it over and did not see a place for him to sign, will we still get our notes??

I watched the video, but it was really confusing with section B. You said as a spousal to leave it blank, but I keep reading on the forum and from here, to fill it with my name, so I don't know.

I did look at your video. It helps. Thank you for posting that up for us.

The online form is whole different challenge all together. I think we are refering to the question on the IMM5744E consent form which is giving us some difficulties.

I was also confused about that second section. My take on it was this. I didn't want them sending the request back to me as incomplete, so I put my information in section B and both my wife and I signed it. Better to have more signatures/information than not enough. They've accepted it and provided the notes to me twice now using that method.

This is where one is sponsoring parents, father and mother - so if spouse ( mother ) signs then they give added info abt her but when its only a husband case - then only his sig is required

For plain spouse cases like most above, the husband or wife signs the consent form, thats all

You cannot order notes from CIC - as you are not Canadian Citizen or PR, in which case only the sponsor can apply which is why you ( the one being sponsored ) give your sponsor the consent to order on your behalf.

If you are the sponsor applying for your self then you order under Privacy Act which is FREE, but you don't get to see any details regarding your sponsorship application

my wife is Canadian citizen and she apply her self but she did not upload any documents and she got 5 doller charges receipt.
she did is correct.

User828 is correct. I have ordered the notes twice and each time I left the Section B blank. The spouse name is required for when there are two applicants (husband and wife) sponsored by a child (PR or citizen).

The consent form must be filed in and signed by the principal applicant. The request for GCMS notes must be done by the Canadian/PR sponsor. In order for them to send the applicants information (Immigration file notes), the applicant must give them consent for releasing such information to the sponsor.

Also, as a side note, the sponsor(if Canadian citizen or PR) doesn't have to be in Canada to apply for the notes. My husband is here with me and all we sent was a copy of his Canadian passport and the consent form.
