its almst 3 months are there still any refusal chances . dint receive medical .what maybe the problm?

Looks from your profile that you applied for your study permit in Islamabad. According to CIC, the processing time for study permit applications in Islamabad is approximately 15 weeks.

In your case, it's only been 12 weeks, I would say wait another 3 weeks and if you didn't hear from them, give them a call.
