How can I apply to get Medical Visits record (not exactly sure if this is what it is called) in Ontario? How soon can I get it? Is there any charge?

hey

Its called Request for Access to Personal Claims History Information. Its free of charge. All you have to fill form and send by post. I think it takes 4 weeks to receive.

It is called OHIP claim history record that you can request from Ministry of health and long term care Ontario.

Last time I did was in 2013 and it took as far as I remember about 6 weeks to get it. You can to fill out their form and mail it to their address. You can google the OHIP claim history form.
