Received PPR: need instructions for sending passport.

I have received my PPR and have to send my passport to the New York Office.

What do I need at this point?

Can someone verify this checklist?

1. Passport valid for 6 months with 2 blank pages
2. 2 photographs with name written at the back
3. Money order of $20 for TT Services fee
4. A prepaid return envelope with address
5. Printout of Passport Request letter

Can I put in my company's email address for return? The mailing system in my apartment is not too good.
