My wife wants to travel to Canada(1st time) from India, she already has a visitor visa(got the visa one year ago) . Does she again need to undergo medical examination? She has done medical on Feb2020 but in the application no re medical examination is showing now.

She probably don't need to undergo another medical examination if she has a valid visitor visa. But she need to take preflight covid test, Find out if she can travel to Canada during covid, as below:
