Quick question: "Copy 1 of medical"

Hello all! This may seem like a stupid question, but can someone explain to me what the "Copy 1 of medical... "
My wife did her examination at a eMedical-enabled clinic, so she has the eMedical reciept, does this mean then I also need a copy of the original as well?

Also, for fees, the reciept for the payment. Copy 2 means? 2 copies?

Did they not give her a copy of the e medical with her photo on it . Also send in the original receipts with it too. e.g. blood work xray receipt . It should have a bar code on the emedical which will help to identify the person and match up with the online medicals. If not call your Dr and get them to send or fax you one over. .

Just send her eMedical receipt that she got from the panel physician. My copy of the medical did not have my photo on it, like taffy7 mentioned. As for the payment receipt, if you are paying online, you will get an email from them with the receipt - print two copies of that and fill in the payer's info, include one copy in the application.
