Quick PPR Qs?

My friend got PPR and have 2 Qs:

(1) Does she has to write her name on both photos? Or is it just a name and address of the studio on one photo only?
(2) On the bio-data page, does she has to write her Canadian child information also?

(1) Does she has to write her name on both photos? Or is it just a name and address of the studio on one photo only? Only on one photo. Details should include: "bear the name and complete address of the photo studio and the date the photo was taken must be included on the back of one photo. The photographer may use a stamp or handwrite this information. Stick-on labels are unacceptable."
(2) On the bio-data page, does she has to write her Canadian child information also? Better provide those details as well.
