Questions Regarding POF, Gift Deed, and Affidavit

Hi all,

My dad is going to sponsor me for POF. I am wondering how to get Gift Deed and Affidavit? Are they something we need to write by ourselves or need authorities to provide?

My dad is in China and will do wire transfer to me. If we need to draft Gift Deed and Affidavit, do we write in Chinese and translate it, or just write in English?

Also, does it need to notarize the translation?
