Just a quick question, is it advisable to put in n/a in the blank areas, if it doesn't apply? I just don't want them to send the form back for something like that.
In some fields, like when it's a "if Yes, explain" and you answered "No" above, no need to put N/A... I never put N/A in my forms, as long as all questions were answered never had any problems though N/A won't hurt
