Questions after getting ITA

Hello Friends,

I have recently received ITA when my score was 449. During the application process, I did not consider my spouse details as it was a tedious process to get her ECA and IELTS.
Now that I have ITA I have few questions:

1. Can I apply for my spouse and kid's PR in the same entry?
2. If not will I have to get my spouses ECA and IELTS done, for sponsoring her PR?
3. Do I need to get the medical assessment for the entire family while submitting my express entry even if they are not coming with me(I am ok to do it if I can apply for their PR along with mine)?
4. Is it necessary for VISA stamping to only happen from the home country or it can be done from the country where you are working?
5. Is it a big problem if you cannot get Police Certificate from all countries you worked in 90 days (I have applied everywhere immediately after getting ITA but chances of getting them all are sparse).

1 - If you include them now your points will get recalculated and if you fall below the cutoff score your application will get rejected.
2 - You have to do your research for sponsorship - this is express entry
3 - Even if they are not accompanying you must provide their medicals and PCC - and if you're a FSW applicant you must have POF for 3 people.
4 - It can happen where you are working.
5 - It will lead to rejection if you do not provide proof that you have applied for such PCC's and that you simply were unable to obtain them within 90 days.
