Questions about IMM 0008

Hello all,

I have a few questions regarding IMM 0008 Generic application form for Canada.

First a little background, I am a Canadian citizen in Toronto applying for sponsorship for my wife and step-daughter from USA.

1. For IMM 0008 my wife is considered principal applicant ... correct ?

2. In Dependants section, it says that ...

You must provide the following details about each of your family members whether they will be accompanying you to Canada or not. You must include your spouse or common-law partner if applicable, and all of your dependent children, and those of your spouse or common.law partner, who are not already permanent residents or Citizens of Canada.

I am confused that in addition to my step-daughter should I also add myself (spouse) ?

3. Under the same section for citizenships, I selected USA as the first one but don't know what to select for second one as she does not have dual citizenship. Should I leave it blank, select unknown or write N/A after printing it ?
