Questions about Forms/Documents for Permanent Residence - PLEASE HELP!!

Hello all,

I have a few questions about the forms I need to fill in and the documents I need to submit for my Permanent Residence application. My questions aren't answered by the on-line instructions, nor the FAQs. My Visa office is the Paris one. It's very frustrating because they only offer an automated response phone number (no possibility of speaking to a human being) and although they promise a contact e-mail address on their website, it doesn't actually appear anywhere (that I can see anyway, and I've just spent a good 10 minutes trawling their website looking for it). Similarly, I cannot contact CIC in Canada as they will only respond to applications being dealt with in Canada. So I've joined this forum hoping some kind soul can help!

So, my situation: I am applying as a Quebec-Selected Skilled Worker (my CSQ was issued a few weeks ago). My ex-common-law partner (we separated 7 years ago) is a permanent resident in Montreal, as is our 9 year old daughter. So, I am the Principal Applicant, and I am not including any other family members in my application.

My questions:

- Form IMM 5406 ("Additional Family Information"): Section A is very confusing. I don't have a current spouse nor common-law partner, but if I leave this blank, I have to sign to say that I "do not have a spouse or a common-law partner, ex-spouse or former common-law partner" - but I do have a FORMER common-law partner. So what do I do, enter her details in the 'common-law partner' box? Maybe with a note to clarify that she is an ex??
- Form IMM 5562 ("Your Travels"): given that I have an EX common-law partner, I assume I don't need to fill in the info for 'common-law partner'?
- Form IMM0008 (Application for Permanent Residence): Glue or staple my photo in?

- Identity and Civil Status Documents: as well as my docs, it asks for copies of "...permanent residence cards for any family members who are permanent residents in Canada". So, this would obviously include my daughter, but does it also include my ex common-law partner?
- Childrens Information: Do I need to provide some kind of proof of custody that my daughter lives with her mother (my ex common-law partner) and if so, what sort of document would this be?
- Travel Documents: As well as mine, do I need to provide the copies of passport pages for my daughter too? And also for my ex common-law partner?

I would really appreciate some answers as I want to submit my application asap!
