Questions about filling out application form

Hello I will be eligible to apply for citizenship in a few months time. I am starting to gather my documents and filling out form CIT 0002 in advance so I can mail my application as soon as I'm eligible. I have the following questions to which I need guidance.

Question # 3B - Request for different name to appear on certificate - is not applicable to me so should I leave it blank or write N/A in the space provided? Should I also do this for all questions that are not applicable to me such as 5B - Mailing address if different from home address?

Question # 3C - List any other names used - on my PR card my name is listed as GivenName(GN) MiddleName1(MD1) MiddleName2(MD2) Surname(SN). However I have used different variations of my name such as GN SN, GN MD1 SN, GN MD2 SN and GN MD1 first initial of MD2 SN. Should I list all the different name variations I have used? And if I do this what should I enter for “Provide details” as all the names are on my passport and PR card? And do you recommend me to attach another page 1 of form CIT 0002 (as there is only space for one name to be listed) or should continue on a blank paper?

I would think, it depends on where you have used those variations. If the name you want to use for your Citizenship certificate is what you have been using for all immigration documents (PR card, passport etc), then I will just use that name again and not border about the variation question.
