Questions about filling out and mailing forms

I am an applicant in China, my husband is my sponsor, and we both living in China now.

1. In the instruction for China, there is an appendix A and a form IMM5604, should I fill out them?

2. When I mail the package, is it right for me to mail the both packages, the sponsorship and the applicant together to Mississagua?

You should fill out all forms that apply to you and ur husband.
Isn't Appendix A the check list?
and you should mail both sponsor and applicant applications together to Mississauga.
Make sure you include all docs that are on the checklist unless they do not apply.
