Questions about document IMM5710 (for Post Grad Work Permit)

Hi there, I have a few questions about the IMM5710 document required to apply for a Post Grad Work Permit. First of all, a bit of information about my situation: I graduated last week from a tourism program at Medicine Hat College and started working at the Royal Tyrrell Museum in Drumheller on May 2, 2016 (the job is part of a 3-week practicum I had to complete to be able to graduate), which will terminate on August 31, 2016, however, there might be an option to stay at this job after that, it's not sure yet. And I don't have a secure job offer after that yet. My current study permit expires on November 30, 2016.

Question 3 (page 1): I don't know which field to choose (A work permit with the same employer, restoration of your status as a worker, an initial work permit or a work permit with a new employer, temporary resident permit).

Details of intended work in Canada (page 3), question 2: I don't have a prospective employer, nor a LMIA, just the current, should I just write then "Not Applicable"?? I also don't get that this question is being asked since the PGWP is supposed to be an open work permit...

And I've been living with my Canadian boyfriend for more than a year, which means that we're a common-law couple, correct? Is there another document I might need to confirm this? We never got anything in writing, just lived together and that's it :)
