I did searched the forum but still a bit puzzled to who must fill in the form for IMM 5406E.
I'm the Canadian sponsor (husband) trying to help my Chinese wife(the principle applicant) to fill in this form. But one requirement that freeze us there going no where by:
Additional Family Information (IMM 5406) – You and each of your family members must complete
their own copy of this form.
I have a 20 year old daughter who's Canadian citizen, but since I divorced many year ago and I don't have contact to my ex-wife and my daughter at all. How can I get them to fill this form?
What if I don't include them to fill the form? will it be considered misrepresentation?
There is a note to the principle applicant saying:
Note: Your present address is the residential address where you and or your dependent(s) are
now physically living. It must be provided in both English and Chinese. If you do not provide an
accurate residential address, it could result in the refusal of your application for misrepresentation.
Should she list my daughter as step-daughter and address as N/A that's all?
Any advice is greatly appreciated.
