Question regarding CIT 0002 Question 11

this is a very confusion question

1. under the address, do i put where i lived, or my work's or educational institution's address,

2. for contact, do i put the work/education contact or simply someone that can confirm that i was there during that time? (such as my husband)

3. do i list all the times i've left on 1-2 week vacations in here as well? (i'm already providing this information on the eligibility calculator document)

1) work/univrsity address
2) someone at your work/university who can confirm you were there
3)vacations not necessary IMO
